Gailliard Adult & Senior Services specializes in Geriatric Care Management services. We understand that many families and caregivers are unprepared for certain situations that life deals us. They feel alone, confused and helpless or just simply do not have the time to deal with the complicated maze associated with care and services needed by an elderly person. Our goal is to provide you with information and connect you to resources that will allow your loved one to live in a safe environment with as much independence as possible. Our job is to coordinate and monitor the process, keeping all information confidential and safe.
Our team is highly trained and specializes in geriatric care management which starts with a comprehensive assessment of the situation. Our Geriatric Care Manager will address issues pertaining to:
medical needs- daily activities
- social needs
- family issues
- legal issues
- safety concerns
- environmental needs
- memory evaluation
- nutritional needs
- depression risks
- financial status
- insurance requirements and more.
After the initial interview and assessment, our managers create a custom plan that will address initial assessment recommendations as well as long term solutions. These recommendations will be re-assessed on a quarterly basis to ensure that all needs and safety requirements are met.
Placement assistance will also be offered, including the assistance with paperwork, and coordination of the placement.
Emergency assistance is also available in crisis situation.
